Job Summary and Qualifications

The Social Worker (BSW) is responsible for promoting patient-centered care by evaluating psychosocial needs and providing community service coordination for identified patient populations. As a member of a multidisciplinary health team, the BSW acts as a liaison between the patient and community resources/post-acute care providers. The BSW creates and facilitates the timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The BSW facilitates patient movement across the continuum of care. The scope of this role includes primarily Med/Surg units.

What you will do in this role:

1. Demonstrates the knowledge and skill to provide psychosocial assessment and discharge planning services to the rehabilitation patient. The Social Worker will not have direct patient care responsibilities but will be knowledgeable of all patients' medical needs.

2. Communicates effectively and professionally with internal and external customers. Documents required information according to hospital policy. Will assist to ensure appropriate orders and documentation are completed concerning the discharge planning needs of the patient.

3. Functions in an organized and time conscious manner. Demonstrates leadership in the Rehab Social Worker role by establishing a communication link among physicians, staff, patients and family members.

4. Collaborates with the interdisciplinary team to coordinate the discharge planning process relative to appropriateness of acute inpatient rehab stay and benefits available. Makes appropriate referrals and utilizes available community/regional resources.

5. Demonstrates a knowledge base of various financial classes and regulations of their respective institutions to facilitate patients through the continuum of care.

6. Performs job tasks to ensure personal, patient, and departmental safety.

7. Presents self in a professional manner.

8. Acquires and maintains current knowledge pertinent to social work practice in rehabilitation ALL ABOVE ELEMENTS ARE CONSIDERED NECESSARY TO ACCOMPLISH THIS JOB.

• QUALIFICATIONSLicensure/CertificationPursuant to Chapter 37 of Title 54.1of the Code of Virginia, Licensure is not required for this position. BLS certification requiredEDUCATIONBSW or MSW degree from an accredited school or university.EXPERIENCEMinimum of 1 year clinical experience in a medical setting with good knowledge of medical terminology and disease processes. Will consider new graduate.SPECIAL QUAUFICA TIONSEffective communication skills. Work is of medium demand, ability to lift, bend, stretch, stoop, etc. Standing and walking most of the time on duty.
